This program is sponsored by a third-party company, Top Notch The Health Center. Top Notch The Health Center and other community partners will sponsor individuals that qualify for the Holistically Positive Program.Holistically Positive covers fees for the required medical evaluation at the designated medical partnership facility, the one (1) year card state fee, along with discounts on products at participating dispensaries. Participants must be 21 years and older and must provide a copy of their current Nevada Driver’s License or State ID Card to submit with the application provided below. Participants will also have to provide medical records for qualifying medical conditions. All applications will be reviewed for eligibility.
